在宅サービス

　在宅で生活する高齢者を介護、援助するのが在宅ケアです。以前は引退後、高齢者用のコミュニティーに移り住む人も多かったですが、最近では「住み慣れた我が家」で最期まで暮らそうという人たちが増えています。

　在宅サービスには様々な種類があり、州・市政府の援助をもらってやっている団体から、民間企業まで、様々な団体があります。また、団体によって提供しているサービス、費用、サービス提供範囲などが異なりますので、いくつかの団体、エージェンシーの担当者に会い、相談をしてみることをお薦めします。その際、確認をするとよいであろうチェック項目は以下になります。
1：事業主、責任者は誰か
2：担当者の経歴は？
3：前向きなプラン、介護計画を立ててくれるか
4：こちらの要望にそった計画を立ててくれているか
5：話を十分聞いてくれるか
6：地域の情報を収集しているか
7：事業主のサービスを押し付けてこないか
8：苦情や相談に素早く対応してくれるか
9：利用者の経済状態を把握したプランを立ててくれるか
10：担当者の担当範囲を超える相談に対し、他団体や他の
　　専門家を紹介してくれるか
11：高齢者の旅行など、介護以外の相談にものってくれるか

　相談をする際、自分の要望がきちんと伝えられるよう、まとめておくことをお薦めします。また、自分の要望と専門家の目から見た「必要であろうサービス」とに食い違いが生じることがあります（例：初期のアルツハイマーの方が、家事援助サービスを週一回、各2時間を希望しているが、専門家としては、それ加えてデイサービスを週二回お薦めする、といったケースや、利用者は脳梗塞の後のリハビリを週6日、一日4時間希望しているけれども、専門家としては週4日、一日2時間で十分で、それ以上は負担が大きくなると判断するなど）。 その場合にはぜひセカンドオピニオンを取り、納得のできるサービスを選びましょう。

Senior Center

　シニアセンターは、コミュニティーに住む高齢者のための核として、ランチサービスや様々なアクティビティーの他、ケースマネジメント（個々人が抱える問題について整理し、必要に応じてサービスのアレンジを行う）、ケースアシスタンス（個々人が抱える問題についての対応をする）、配食サービス、栄養指導といった様々なサービスが提供されています 。それぞれのセンターによって、提供するサービスは異なりますので、ご注意ください。

<デイ ・ケア　サービス>
Adult Day Healthcare Service, Adult Day Healthcare Programs, etc.

　日本では「日帰り通所施設」とも呼ばれ、身体的に虚弱になった方、認知症（アルツハイマー症、脳血管性認知症など）の方などを対象とし、リハビリテーションや音楽、手芸、ダンスや討論会といったアクティビティーを提供しています。

　センター・施設により、送迎サービスがあるので、一人で外出することが難しい方でも参加でき、また、施設にはソーシャルワーカーや介護職員がいますので、気軽にいろいろな相談にのってもらうことができます。

　デイサービスを検索する際のチェックポイントは以下になります。
1：場所は自宅から近いか。
2：送迎バスはあるか。ある場合、福祉車両（車椅子対応で、
　  座席にシートベルトが着いている）か。
3：地域とどれだけ密接に結びついているか。
4：利用者や家族の会があるか。
5：日常の生活を重視しているか。
6：緊急時の対応はどうなっているか。
7：職員は気さくで丁寧か。態度が砕けすぎていないか。
8：オムツの交換や薬の投与などに対応してくれるか。
<配食サービス>
Meals on Wheels

　シニアセンターやナーシングホーム、民間のエージェンシーなどが、ご自宅まで食事をお届けするサービスです。食事は一日一回、調理されたものが届いたり、一週間に一度、冷凍になった食事がまとめて届けられるなど、団体によって形式が異なり、また、費用も＄1程度の低料金のものから一ヶ月で＄300程度かかるものなど、いろいろです。

　在宅看護・介護・家事援助のサービスは、それぞれの専門家がご自宅を訪問し、必要なサービスを提供するものです。在宅サービスは在宅で生活する高齢者をサポートすることにあり、食事を作る援助が必要な方には料理を、室内の整頓が必要な方には掃除を、会話が必要な方には会話を、と、派遣されるお宅によってサービス内容は異なり、派遣を希望される人（クライアント）と派遣される人（団体、エージェンシーまたは個人）との契約によって仕事内容が決まります。

　日本と異なり、団体・エージェンシーによって提供するサービスに違いがあり、また、皆様がお受けになりたいサービスによって、訪問する専門家の必要資格が変わるなど、細かな規定があります。
サービス内容の違いは以下になります。

*費用は、ライセンス（RN, PT, OT, ST, CAN, CHHA, etc.）の有無、保険（業務中の事故に対する保障）の有無、派遣先の施設、団体、エージェンシーの手数料によって異なります。

　サービスを利用するに当たっては、1.派遣される人のリファレンスが取れるか、2.派遣される人が気に入らなかった場合、変えてもらうことはできるか、3.派遣される人の犯罪歴、セクシャルハラスメント、薬物使用歴などのチェックをきちんと行っているか、を確認することをお薦めします。

　看護・介護・家事援助サービスは費用的に決して安いものではありませんが、自分で無理をして怪我をしてしまったり、寝付いてしまうよりも、払うべき費用を払って専門家を雇ったほうが、最終的に安く付く場合があります。また、特に一人暮らしの方の場合、家事援助サービスを利用し、定期的に人が訪ねてくる、という状況を作ると、身だしなみに気をつけたり、一緒に買い物に出かけたりすることで社会との接触を保ち、引きこもりや痴呆を防ぐといわれています。

看護・介護・家事援助サービスを利用する際のチェックポイントは以下になります。（団体、エージェンシー、個人派遣共通）
1：他の派遣先の体験や秘密を世間話として話していないか
2：買い物の精算など、お金の扱いはしっかりしているか
3：サービスを受けたあと、物がなくなっているということはないか
4：万が一、物をこわしてしまった（例：コップを割ってしまった）
　  時に正直に話すか。団体、エージェンシーは対応してくれるか
5：時間に正確か。遅刻、早退はないか
6：風邪をひいた状態で訪問していないか
7：余計な介護用品や健康食品などを買わせようとしないか
8：高齢者特有の病気に関する知識があるか
9：家族や緊急連絡先への連絡・報告・相談ができているか
10：（担当者が）要望を伝えやすい人柄か

＜ショートステイ（Respite Care）＞
　在宅で看護・介護を受けている高齢者が、家族や介護を担っている人の休養、旅行や急な用事のために、一時的に施設に短期入所するサービスです。短期入所専門の施設はほとんどなく、ナーシングホームやアシスティッドリビングなど、施設サービスを行っている団体が行っている場合がほとんどです。そのため、施設の特色がそのままサービス内容の違いとなります。ショートステイを行っている団体は数が多くないので、利用を考える方は早めに担当者とコンタクトを取ってください。

ショートステイ利用の際に、確認しておくべきチェックポイントは以下になります。

1：入所中のすごし方について、相談や提案があるか
2：施設は清潔か
3：生活実態があっているか（スケジュールや食事など）
4：風邪などをひいても、利用できるか
5：利用者個人の尊厳、プライバシーが保たれているか
6：スタッフがフレンドリーか
7：他の入所者は、楽しそうか
